What to bring to your Energy Assistance appointment:

Income

• Gross income from all sources for the past 4 weeks
• This includes employment, child support, alimony, unemployment, workers compensation, cash gifts

To you get SNAP, Cash or TANF?

• Your DSS award letter

Self-Employed?

• Last income tax return- all schedules
   

Assets

• Bank Statement on bank letterhead that show transactions for checking, savings, IRAs, etc.
   

Electric

• Most recent electric bill- not a shutoff notice
   

Monthly Housing Cost

• Renters- Contact information for your landlord, plus your most recent rent receipt or a cancelled check or recent lease
• Homeowners-Most recent mortgage statement, cancelled check, letter from mortgage company stating monthly payment due on letterhead

Other information

• Social Security numbers and birth dates of all household members.
